Our Transitions Neurological Day program stresses functional and practical individual and group therapy. Organized in two tracks, the "Home Alone" track focuses on gaining independence in the home setting, while the "Community Re-Entry" track focuses on successful reintegration into the community.

To make a referral, please contact our program coordinator at 202.877.1945. Referrals can be made by physicians, other health care professionals, community agencies, case managers, insurance companies, hospitals, family members or oneself.
